The form is available in two formats: Microsoft Word^TM and Adobe Acrobat_TM

If you use Microsoft Word, download the Word formatted file.  If not, download the Acrobat formatted file  The latter will require you to have the Adobe Acrobat Reader.  If you don't already have it, you can download it for free from the Adobe site Here 

When you have download the application you need, print it out, fill it in and return it to the address on the form with your payment. Or, if you wish, you may fax the form to the IFSA office. Their fax number is: 262-782-2092
